Documents Required for Assistance
Please call us at (812) 753-4964 and schedule an appointment. Applications for assistance are in the mailbox by the front door.
Please bring any and all of the following information and verification that applies to your situation for each of your household members for the last 30 days when coming to the offices for assistance.
The following documents are required for all financial assistance:
- Tax Return
- Identification (driver’s license, birth certificate)
- Bank Statement
- Social Security or Disability statements showing deposits
- Check stubs if employeed
- Bills
- Application (available in the mailbox by the front door 24 hours a day. The image to the right shows the mailbox location. —>
- Verification of property taxes and homeowner’s insurance
- Renter’s insurance verification (receipts, statement)
- Utility bills including gas, electric, water, sewer, telephone, and cable
- Credit cards and other monthly obligations
- Life Insurance policies
- Health/Medical insurance information (insurance card
Other required documents may include:
- Copy of lease
- Landlord’s name, address and phone number
- Mortgage or contract payment book
- Check stubs, award letters, or other information pertaining to any income you may have (employment, support, benefit income, contributions, etc) or termination of income
- Verification of any property (land or buildings) which you own and are not living on (tax receipts, mortgage papers, deeds, notes)
- Divorce decree; support orders
- Bank statements: checking/savings accounts, certificates of deposit, IRA’s, 401K’s (Printed out from bank for the last 30 days)
- Non-U.S. citizens: visa or permanent alien card
- Social Security card(s) for all members of the household
- Medical expense verification
- Verification of child care and dependent care costs
- Vehicle — own/lease statement and registration
